About Trail Lake Nursing & Rehabilitation
Welcome to Our Family
Through the years, our staff has been dedicated to meeting and exceeding our goal of providing uncompromising quality care to our residents and their loved ones. Many of our staff members have been here long enough to consider this facility as home. You can count on us being here for you for as long as you require, no matter what your need or care might be.
About Us
Here at Trail Lake Nursing & Rehabilitation, we offer a living alternative for those unable to care for themselves, and a wide array of nursing and rehabilitation services for short-and long-term stays. Our staff is committed to providing exceptional care for your family. From the admission of the first resident seeking help many years ago to today our philosophy has always been and remains that living and rehabilitation must include caring for the mind, body and soul. When you can’t live or recover in your own home, we invite you to ours.
CARE Program
What is CARE? CARE is our company culture and guides the approach we take with our residents, families, each other and the community. Each letter represents a word that reminds us of our focus on Connecting, Acting, Recognizing and Enhancing the lives of our residents and the community.
Who We Care For
Trail Lake Nursing & Rehabilitation is a licensed nursing facility in southwest Fort Worth that has served the community since 2007. We care for two groups of people. The first are patients who come to us from the hospital for short-term rehabilitation, usually after joint replacement, a fracture, a stroke, heart or lung problems, or a long illness that has left them weak. The second are residents who need long-term care because their health no longer allows them to live safely at home. Many people need a mix of both, and our team adjusts the plan as needs change.
Our Care Team
Good care depends on the people who give it. At Trail Lake, each resident is supported by a team that includes:
- Licensed nurses and certified nurse aides on duty 24 hours a day, providing skilled nursing care
- A Medical Director and pharmacy consultant who oversee medical care and medications
- Licensed physical, occupational and speech therapists
- A full-time dietary manager supported by a registered dietitian consultant
- A full-time certified activity director
- A social worker who provides emotional support, care planning and discharge planning
We staff above state-mandated levels based on the care our residents need, and every employee completes a thorough background check before joining the team.
Life at Trail Lake
A nursing facility should still feel like home. Residents enjoy freshly prepared meals with separate, independent and assisted dining options, a full-service salon, wireless internet, and a calendar of activities that includes gardening, music, arts and crafts, games, cooking and religious services, along with monthly outings. Family members are welcome to visit at any time, as long as visits respect a roommate's rest and privacy, and our landscaped courtyard is a favorite spot for time outdoors. Our CARE Culture in Practice
CARE is more than a slogan on the wall. It shapes the small, everyday moments that make up a resident's day:
- Connecting means taking time to learn each resident's story, routines and preferences, and keeping families closely involved in care.
- Acting means responding promptly when a resident needs help or when health changes, and following through on what we promise.
- Recognizing means celebrating residents and staff, from milestone birthdays to the daily progress made in therapy.
- Enhancing means looking for ways to make each day better through good food, meaningful activities, comfort and dignity.
Our History in Fort Worth
Trail Lake Nursing & Rehabilitation opened in the spring of 2007 as a new skilled nursing and rehabilitation facility in southwest Fort Worth, with a strong focus on helping rehab patients return home. Over the years the building has also become a long-term home for many residents, and several have celebrated remarkable milestones here, including Louis D. Cass and Roberta Evens, who both marked their 100th birthdays with the Trail Lake family. In 2017, operations transitioned to a Fort Worth-based operator, and residents continued to see the same familiar faces providing personalized care.
Licensed, Certified and Accountable
Trail Lake is licensed and certified by the Texas Health and Human Services Commission and is certified to take part in the Medicare and Medicaid programs. Families can review our federal inspection and quality information on Medicare's Care Compare website. Residents and families also have access to a free, confidential long-term care ombudsman at (800) 252-2412, who can help resolve concerns.
