Care Options at Trail Lake
Short-term rehabilitation
After surgery, a fall, a stroke or a serious illness, many people are not quite ready to go straight home. Our short-term rehab program in Fort Worth combines physical, occupational, speech and respiratory therapy with 24-hour nursing, so you can rebuild strength and independence and get back to the life you had before.
Skilled nursing care
Some medical needs call for licensed nurses around the clock: wound care, IV therapy, diabetes management, tracheostomy care or careful medication management. Our skilled nursing care is supported by a Medical Director, a pharmacy consultant and on-site access to laboratory, radiology, podiatry and dental services.
Long-term care
When living at home is no longer safe, long-term care at Trail Lake provides help with daily activities, nutritious meals, social services, a full calendar of activities and the reassurance of nursing staff on duty day and night. Private and semi-private rooms are available.

Why Families Choose Trail Lake
- Licensed and certified. Trail Lake is licensed and certified by the Texas Health and Human Services Commission and participates in Medicare and Medicaid.
- Nursing around the clock. Licensed nurses and nurse aides are on duty 24 hours a day, and we staff above state-mandated levels based on the care our residents need.
- Therapy under one roof. Licensed physical, occupational and speech therapists work with each patient on goals that matter to them, from walking safely to swallowing and speaking clearly.
- Whole-person care. Our CARE culture (Connecting, Acting, Recognizing and Enhancing) guides how we treat residents, families and each other. Learn more about Trail Lake.
- Everyday comforts. Meals planned by a full-time dietary manager with a registered dietitian consultant, a full-service salon, wireless internet, daily activities and monthly outings.
Paying for Care
We accept Medicare, Medicaid and private pay, and we will bill third-party insurance. For eligible patients, Medicare Part A can cover a skilled nursing stay after a qualifying hospital admission, while Texas Medicaid can help pay for long-term nursing facility care. Our admissions guide explains how each option works in 2026 and what to bring on the day of admission.
Choosing a Nursing Home or Rehab Center
Families often have only a few days to choose a rehab center when a hospital discharge is coming up. A little preparation helps. Whether you visit Trail Lake or another facility, these questions are worth asking:
- What kind of care does my loved one need? A few weeks of therapy, ongoing nursing for a medical condition, or long-term help with daily living? The answer points you toward the right program.
- How is therapy scheduled? Ask how often therapy happens, which therapists are involved and how goals are set and reviewed.
- Who is on duty at night and on weekends? Skilled patients need licensed nurses around the clock, not only during office hours.
- How will we stay informed? Ask about care plan meetings, who your contact person is and how families are told about changes.
- What will it cost? Confirm which payer will cover the stay and what you may owe, especially after day 20 of a Medicare-covered stay.
- How does it feel? Visit at a mealtime or during an activity if you can, and notice how staff talk with residents.
Families can also look up any certified nursing home's inspection results and quality information on Medicare's Care Compare website. We welcome those questions and are glad to answer them on a tour.
