Trail Lake Nursing & Rehabilitation is at 7100 Trail Lake Drive, Fort Worth, TX 76133, half a block from the intersection of Trail Lake Drive and Altamesa Boulevard.

• From I-20: exit at Trail Lake Drive and head south for about one mile to the intersection with Altamesa Boulevard. Continue straight through the intersection; we are half a block ahead on the right.
• From I-35W: exit at Altamesa Boulevard and head west to Trail Lake Drive. Turn left onto Trail Lake Drive; we are half a block ahead on the right.
